Are you passionate about supporting young people with Special Educational Needs and Disabilities (SEND) to achieve successful outcomes and progress into adulthood?

We are looking for an Inclusive Transition Coordinator to join our Student Services/SEND team. In this rewarding role, you will support students with SEND through their transition into college, helping to ensure they have the right support in place from the very beginning of their journey. Working closely with schools, families, curriculum teams and external agencies, you will play a key role in identifying individual needs, coordinating support and promoting positive progression opportunities.

This is an excellent opportunity for someone with experience of supporting young people with SEND who is committed to inclusion, independence and ensuring every learner has the opportunity to succeed.

Key Responsibilities

  • Work collaboratively with schools and education providers to support prospective learners with SEND as they transition into college.
  • Assess individual learner needs and develop personalised, person-centred transition plans.
  • Manage a caseload of learners, maintaining regular contact and monitoring progress against agreed actions.
  • Contribute to Education, Health and Care Plan (EHCP) consultations, reviews and transition planning activities.
  • Liaise effectively with learners, parents/carers, curriculum teams, support services and external agencies.
  • Coordinate transition activities, including visits, familiarisation sessions and information-sharing meetings.
  • Ensure learner support information is communicated accurately and securely to relevant college teams.
  • Maintain accurate and confidential records in line with data protection requirements.
  • Promote inclusion, independence, high aspirations and positive progression outcomes for learners with SEND.
  • Support preparation for adulthood outcomes, including employment, independent living, community participation and health.

Essential Criteria

  • Level 2 or Level 3 qualification in learning support, SEND, education, youth work or relevant equivalent experience.
  • Level 2 qualifications in English and Mathematics (or equivalent).
  • Experience of supporting young people with SEND within an education, training or related environment.
  • Knowledge of SEND and Education, Health and Care Plans (EHCPs), with the ability to develop person-centred support plans.
  • Excellent communication, organisational and relationship-building skills, with the ability to manage a varied caseload.
